The Community Support Discharge Programme offers local people and their families the opportunity to help the elderly in their community.

As a Community Health Volunteer you will ensure that our elderly neighbours do not stay in hospital any longer than necessary.

Our Mission

To work directly in partnership in the community with statutory health and social care services. To provide additional support through volunteers based in the community at points in a person’s life where a short intervention could reduce the risk of a longer stay in hospital or hospital readmission.

 

As a volunteer, once a patient is discharged from hospital you will encourage them in their recovery alongside the partnerships START team. By offering social and practical support, collecting prescriptions, basic shopping, popping in for a chat and possibly assisting them with their gentle exercises.

It is important to make sure that people stay connected to their family, neighbours and general community. We believe if people feel valued, feel they have purpose and a sense of belonging they flourish and stay well.
